XB_EC_12 not automatically going to OP on startup

Hello,

I have a customer with a CtrlX Core X3 (2.04) and is using only the EtherCAT master and PLC App for an application. They have an XB_EC_12 with a digital In and digital out module only. The application is working, except that when they power cycle the system, the IO modules stay in INIT phase even though the EtherCAT master says that the requested state is OP. They are able to get online with the controller and request it to go to OP mode again, the IO modules go to OP mode just fine. There is no error on the Logbook that would indicate why the units don't go straight into OP mode automatically, and they work right away when we command it to go to OP mode. I'm unsure if there is a configuration missing or that needs updating.

The IO modules and ESI files were originally for XB_EC_12 firmware version 1.2.0 with Revision 0x0000 0100. We updated the firmware to 1.4.0 and ESI file to revision 0x0010 0100 just to see if that was a problem but that didn't resolve anything. We also re-applied the System Image for 2.04 to the Core and reloaded the apps. We rescanned the bus with IO engineering and reloaded the EtherCAT configuration, but with that it still didn't automatically go into OP mode on powerup.

We generated a short-term solution in the PLC to check the EtherCAT master state and command it into OP mode if it isn't on powerup, and that has gotten the customer out of being stuck for the moment. But, I want to determine the reason they are having this as I don't know what is causing this issue and whether there is a step we are missing. Can anyone assist in this?
